Shoulder Pathology and Post-Operative Shoulder Pain: Where Acupuncture Enhances Physiotherapy Pathways
Shoulder pathology and post-operative shoulder pain represent complex clinical conditions in which peripheral tissue injury, neurogenic inflammation, segmental spinal mechanisms, and central sensitization interact to impair functional recovery.
Physiotherapy remains the cornerstone of shoulder rehabilitation, aiming to restore tissue loading, motor control, and functional integration. However, pain-related motor inhibition, altered afferent input, and maladaptive neuroplasticity frequently limit physiotherapy effectiveness, particularly in the acute and post-operative phases.
Within a biomedical framework, acupuncture modulates neuro-musculoskeletal pathways that directly intersect with rehabilitation mechanisms. Peripherally, it influences nociceptive and inflammatory signaling within muscles, tendons, and periarticular tissues. Segmentally, acupuncture modulates spinal nociceptive processing within cervical segments related to shoulder innervation, reducing reflex motor inhibition. Suprasegmental effects include engagement of descending inhibitory systems and modulation of pain-related brain networks, potentially improving movement tolerance and motor relearning.
In post-operative settings, acupuncture has been associated with reduced analgesic and opioid requirements, facilitating earlier mobilization without pharmacological motor suppression. By supporting neurobiological conditions favorable to tissue healing and movement, acupuncture functions as a physiotherapy-enhancing modality. This lecture will focus on evidence-based acupuncture techniques and the MUAPs (Most Useful Acupuncture Points – Karavis et al., 2025) applied in acute pain, chronic pain, tissue healing, and rehabilitation.
